Online pharmacies have been one of the fastest growing sections of the pharmaceutical industry over the last few years, through a mixture of lower costs for the customer and lower operating costs. However despite this, there are many people who have called for tighter regulations to be placed on online pharmacies. As well as the fact that they seemingly have several shortcomings when compared to regular in store pharmacies.

The truth of the matter is as usual neither one side or the other, with online pharmacies having several advantages as well as drawbacks against conventional services. One aspect of online ordering that is often something of a gamble is that many of the drugs that you can buy on certain sites aren’t necessarily FDA approved yet and might carry severe side effects. Without going through the process of discussing what you need with your doctor, getting new and experimental drugs can sometimes be dangerous. On the other hand this is also a good way to get drugs you otherwise might not be able to find, particularly if you know enough about them and what you are able to take safely.

The other advantage that online pharmacies have of course is that they deliver whatever you order straight to your door without any trips into town. This is particularly useful if you are in a fairly remote location, or live somewhere that doesn’t have a large pharmacy nearby, particularly if you need an obscure or not widely kept drug. This is also a good thing for the sick or elderly who perhaps cannot easily get around even short distances.

Despite all the apparent advantages of online pharmacies however, there are several drawbacks that should always be noted before you use one. The main disadvantage of course is that the delivery of the items you buy can take several days and in some cases even a couple of weeks. Which of course isn’t always much good if you are running out and haven’t thought ahead and ordered early. Conventional overnight script filling is another thing that cannot usually be done in this way, although there are now a few companies who guarantee delivery in as little as 48 hours.

The other main disadvantage of online pharmacies is the fact that you don’t get the personalized service and advice that you can get with in store pharmacies. Pharmacists will often offer advice on treatments, and even different and cheaper drugs to their customers. This is rarely possible online however, and often the online companies have few if any qualified pharmacists that you can talk to if you have questions or concerns.

Some sites offering pharmaceuticals have recently been found to either be selling unsafe medications, or to have been hoaxes designed to steal customers money and not deliver any drugs at all. These sites are small in number, but the fact that they exits means that being careful when ordering online is even more important than usual. Of course these risks are lessened a lot by conventional pharmacies, and if anything suspicious does happen, then you at least know the source.

What is Low Back Pain?

Low back pain can affect the back anywhere below the ribs and above the legs. The lower back is the connection between the upper and lower body, and it bears most of the body’s weight. Because of these roles, it is easily injured when you lift, reach, or twist.

Almost everyone has low back pain at one time or another. The good news is that most low back pain will go away in a few weeks with some basic self-care. But if your pain is severe or lasts more than a couple of weeks, see your doctor.

What causes low back pain?
Low back pain is often caused by overuse, strain, or injury. For instance, people often hurt their backs playing sports or working in the yard, being jolted in a car accident, or lifting something too heavy.

Aging plays a part too. Your bones and muscles tend to lose strength as you age, which increases your risk of injury. The spongy discs between the bones of the spine (vertebrae) may suffer from wear and tear and no longer provide enough cushion between the bones. A disc that bulges or breaks open (herniated disc) can press on nerves, causing back pain.

In some people, low back pain is the result of arthritis, broken vertebrae (compression fractures) caused by bone loss (osteoporosis), illness, or a spine problem you were born with.

Often doctors don’t really know what causes low back pain. But it is more likely to become long-lasting (chronic) if you are under stress or depressed.

What are the symptoms?
Depending on the cause, low back pain can cause a range of symptoms. It may:

Be dull, burning, or sharp.
Be felt at a single point or over a broad area.
Come on gradually or suddenly.
Occur with muscle spasms or stiffness.
Cause leg symptoms, such as pain, numbness, or tingling, often extending below the knee. These symptoms can occur on their own or along with low back pain. Leg symptoms are often caused by lower spine problems that place pressure on a nerve that leads to the leg.
A rare but serious problem called cauda equina syndrome can occur if the nerves at the end of the spinal cord are squeezed. Seek emergency treatment if you have weakness or numbness in both legs, or loss of bladder or bowel control.

Doctors say back pain is:

Acute if a spell (or episode) of pain lasts less than 3 months. Most back pain is acute and goes away with 4 to 6 weeks of home treatment.
Recurrent if acute symptoms come back. Most people have one or more episodes of recurrent low back pain.
Chronic if your back bothers you most of the time for longer than 3 months.
How is low back pain diagnosed?
The doctor will ask questions about your past health, symptoms, and work and physical activities. He or she will also do a physical exam. Your answers and the exam can help the doctor rule out a serious cause for the pain. In most cases, doctors are able to recommend treatment after the first exam.

Most people do not need further testing. Imaging tests such as X-rays, CT scans, and MRIs are not helpful for diagnosing most episodes of low back pain. In most cases, they are only used if the doctor suspects a serious problem, such as a herniated disc, a broken bone, or cancer, or if surgery is being considered or planned. You might also have imaging tests if worker’s compensation or a lawsuit is involved.

How is it treated?
Most low back pain will improve with the following treatment:

For the first day or two, rest in a comfortable position. Try lying on your side with a pillow between your knees. Or lie on your back on the floor with a pillow under your knees. Do not stay in one position for too long, though. Every 2 to 3 hours, take a short walk (about 10 to 20 minutes), then rest in a comfortable position again.
Take over-the-counter pain medicine if needed, such as acetaminophen (Tylenol, for example) or an anti-inflammatory drug such as aspirin or ibuprofen (Advil or Motrin, for example). For most people, these medicines work best if taken on a regular schedule.
Try using a heating pad on a low or medium setting for 15 to 20 minutes every 2 or 3 hours. Try a warm shower in place of one session with the heating pad. You can also buy single-use heat wraps that last up to 8 hours. You can also try an ice pack for 10 to 15 minutes every 2 to 3 hours. There is not strong evidence that either heat or ice will help. But you can try them to see if they help.
As soon as possible, get back to your normal activities. Staying in bed for more than 1 or 2 days can weaken your muscles and make the problem worse.
Walking is the simplest and maybe the best exercise for the lower back. It gets your blood moving and helps your muscles stay strong. Start with easy walks of 5 to 10 minutes a day, and gradually increase your time. Walking in water up to your waist or chest is also good exercise.

A doctor or physical therapist can recommend more specific exercises to help your back muscles get stronger. These may include a series of simple exercises called core stabilization. The muscles of your trunk, or core, support your spine. Strengthening these muscles can improve your posture, keep your body in better balance, and lower your chance of injury.

Some people get relief from pain by using treatments such as massage, spinal manipulation (chiropractic or osteopathic manipulation), or acupuncture. Certain treatments work for some people but not for others. You may need to try different things to see which work best for you.

If your symptoms are severe or you still have them after 2 weeks of self-care, see your doctor. You may need stronger pain medicines, or you might benefit from physical therapy.

Having ongoing back pain can make you depressed. In turn, depression can have an effect on your level of pain and whether your back gets better. People with depression and chronic pain often benefit from both counseling and medicine. A cognitive-behavioral therapist can teach stress management and pain control skills. Antidepressant medicines may help too.

Only a few people with low back pain need surgery. Surgery may help if you have a herniated disc or back pain along with symptoms of nerve damage, such as numbness in your legs. Even in these cases, most people will improve without surgery. Having surgery does not guarantee that all your pain will go away. Before you have surgery, it is a good idea to get a second opinion.

How can you prevent low back pain from returning?
After the first time you have had low back pain, you are likely to have it again. To help keep your back healthy and avoid further pain:

Practice good posture when you sit, stand, and walk.
Get regular, low-impact exercise. Walk, swim, or ride a stationary bike. Stretch before you exercise.
Wear low-heeled shoes with good support.
Sleep on your side. A medium-firm mattress may put the least stress on your back.
Watch your weight. Being too heavy, especially around your waist, puts extra stress on your back.
Don’t try to lift things that are too heavy for you. When you must lift, bend your knees and keep your back straight, keep the object you are lifting close to your belly button, and avoid lifting and twisting at the same time. See a picture of proper lifting technique.
If you sit or stand for long periods at work:

Pay attention to your posture. Sit or stand up straight, with your shoulders back.
Make sure your chair has good back support.
Take regular breaks to walk around.
If your work involves a lot of bending, reaching, or lifting:

Talk to your human resources department to see if there are other ways you can do your work.
Don’t depend on a “back belt” to protect your back. Studies have not shown these belts to be effective in reducing back injuries. The most they can do is to help remind you to use good techniques for lifting.

Fibromyalgia is a chronic condition whose symptoms include muscle and tissue pain, fatigue, depression, and sleep disturbances; it often mimics or appears together with other chronic conditions such as irritable bowel syndrome (IBS), lupus, and arthritis. “There are usually other things that are triggering it, so you have to individualize treatment for each patient,” says Daniel Arkfeld, MD, an associate professor of clinical medicine at the Keck School of Medicine at the University of Southern California. Because it is such a complex condition, fibromyalgia is difficult to treat. Medication tends to reduce symptoms only by about 30% to 50%. Many patients seek diet and lifestyle changes as treatment; in one survey, 42% of patients reported that certain foods aggravated their symptoms.

How Does Food Affect Pain?
Recent data suggests that central sensitization, in which neurons in the spinal cord become sensitized by inflammation or cell damage, may be involved in the way fibromyalgia patients process pain. Certain chemicals in the foods patients eat may trigger the release of neurotransmitters that heighten this sensitivity, Dr. Arkfeld explains. Because of this, fibromyalgia patients often have food sensitivities that may not show up in food allergy tests.

Although there have only been a handful of studies on diet and fibromyalgia—with no definitive links found—experts suggest that the following 10 healthy eating rules can’t hurt, and may help, people dealing with chronic pain.

Fresh Foods, Mostly Plants

Because many fibromyalgia patients have food sensitivities, eating a diet of fresh foods, devoid of preservatives and additives, may ease symptoms triggered by coexisting conditions such as IBS, according to Dr. Arkfeld. Several studies have shown improvement of symptoms in fibromyalgia patients who followed vegan and vegetarian diets, although most of the patients in those studies chose to discontinue the diets after the study. Simply cutting back on the amount of red meat in your diet may help as well.

It’s also a good idea to buy organic food when possible, says Dr. Arkfeld. “Some patients do better avoiding pesticides and chemicals.”

Lay off the Caffeine

Fibromyalgia is believed to be linked to an imbalance of brain chemicals that control mood, and it is often linked with unrestful sleep and fatigue. Fibro patients may try to ease fatigue with stimulants like caffeine, but they may end up doing more harm than good in the long run.

“Caffeine is a loan shark for energy. We recommend not using a lot,” says Kent Holtorf, MD, the founding medical director of the Fibromyalgia and Fatigue Centers, which are located across the country. He explains that though caffeine provides an initial boost of energy, it is no substitute for sleep; in fact, it can stay in your system for several hours and keep you awake long after you’ve finished a cup of joe.

The Veggie Paradox

“For joint pains, a lot of people say avoid nightshade vegetables,” says Dr. Arkfeld. Normally veggies are low-cal, healthy options, but the nightshade variety—including tomatoes, potatoes, and eggplant—are suspected to trigger arthritis and pain conditions in some people. “The thought is that they have components that are neurotoxins. For a small percentage of patients, cutting them out makes a dramatic difference,” says Dr. Holtorf. Though there is anecdotal evidence of these foods exacerbating symptoms, research hasn’t proven these plants to be harmful.

For a lot of Americans today, keeping up with their prescription drug bills is proving a challenge. A few Americans have zero prescription coverage, while others have never been able to afford medical coverage to begin with. The fact that our pharmacy insurance pays for our prescriptions drugs if we have coverage, has never really made us think about why American pay so much more for medication than anywhere else in the world. A little research reveals that it’s not as much of a safety issue as the government wants you to think, but more of a financial scandal.

The American government would have you believe that prescription medication from anywhere other than the United States are not safe, no matter where they are from. This is simply not true. Many other countries such as Canada and Great Britain have just as strict of safety regulations and high quality standards for their medications, the only difference is they pay much less for theirs.

The reason Canadians can afford their medications is because their government imposes price controls on their medications. This means that drug companies cannot charge more for a particular drug than the market dictates is fair. Whereas here in America, we allow drug companies to have patents and periods of market exclusivity for too long which allow drug companies to charge as much as they want for a particular drug.

Interestingly enough, although the US government maintains that safety is the main issue for drug importation to be discouraged, many government programs actually promote buying drugs from foreign countries. is drugs from overseas and Canada are not of  inferior quality, but American drug companies pay the government well to keep an unfair monopoly on the US prescription drug market. It may just well be that big government may not be as concerned about the health and safety of ordinary Americans as much as they are concerned about money from big pharma.
